Payer–Provider Collaboration Advances Care

WESTCHESTER – MVP Health Care (MVP) and Westchester Medical Center Health Network (WMCHealth) recently announced a next‑generation strategic partnership designed to improve care delivery by bringing the health plan and hospital teams together at the point of care. This model enhances support of MVP members during hospitalization and as they transition to recovery.

Through the partnership, MVP’s clinical care managers work alongside Westchester Medical Center (WMC) care teams within the hospital and support members as they transition home and navigate follow‑up care and services. Rather than operating as separate organizations with separate processes, MVP and WMC will function as a connected care team, aligning support in real time across the care experience. By bringing care coordination directly into the clinical setting and extending that support beyond discharge, the partnership helps reduce gaps between hospital and home by better aligning care plans with each member’s clinical needs with their insurance coverage to support stronger outcomes.
